The application package has been completed and you have confirmation of admission from Crossroads Centre.

Stabilized from withdrawal symptoms prior to admission.

Travel arrangements are confirmed if you are arriving from outside of Thunder Bay.

Child care arrangements have been made for the duration of your admission.

How to apply

You can download our Application Form and Consents by clicking HERE. It is a fillable PDF form that you may complete electronically, or you can print and complete it by hand. If you are unable to download or print the Application Form, a copy can be sent to you electronically, by mail, or by fax upon request.

It is important that the Application Form be fully completed and signed to avoid delays in the application process. Once fully completed, the Application Form can be submitted by Email, fax, mail or in person.

If applicable, please include a copy of any current probation or parole conditions with your application (we do not participate in mandated treatment as a means of early release).

Upon receipt of the completed Application Form, the Intake Coordinator will be in contact with you to discuss the application and arrange a time to meet in person or by telephone to complete the application process.

Personal information shall not be used or disclosed for purposes other than those for which it was collected, except with the consent of the individual or as required by law.  Personal information shall be retained only as long as necessary for the fulfillment of those purposes.

WELCOME AND ADMISSION INFORMATION

WHEN YOU ARRIVE

Welcome!  The admission process will begin as soon as you arrive.  We will retrieve your medication and store any item/s that need to be secured.  After admission, you will be shown to your room and have time to unpack, get settled in and meet your peers before orientation.

CELL PHONES & ELECTRONICS

Cell phones are allowed with restrictions.  Laptops/tablets and other electronics will be secured and returned for use off-site, or for specific purposes related to your recovery goals only.

WHAT TO BRING
  • 1 – 2 weeks’ worth of clothing and all personal items must fit in 2 bags/small suitcases.  Excess belongings will be stored for up to 30 days while you arrange to store these off-site.  You are also responsible to arrange to store acquired items that do not fit in the storage space provided off-site.
  • Clothes for recreation/physical activity.
  • Sleepwear.
  • Outerwear depending on the season.
  • One pair of athletic shoes, one pair of outdoor footwear, slippers.
  • Toiletries (i.e. soap, shampoo, toothpaste, toothbrush, comb/brush, unscented lotion).
  • Hairdryer.
MEDICATION

You will need to arrange to have 4 weeks of refills on your prescriptions before admission. We require all medication be received directly from a pharmacy, treatment program to healthcare facility.  If you are arriving from home or another location we will arrange to have your prescriptions transferred and delivered after admission.

WHAT IS PROVIDED
  • All meals.
  • Twin bed, bedding, pillow, towels.
  • Closet, clothes hangers.
  • On-site laundry.
  • Shared accommodation (2 people to a room); there are limited single rooms and these are assigned to individuals who have completed treatment and core programming.
WHAT NOT TO BRING
  • Personal bedding (i.e. pillow, blankets).
  • Mouthwash, hand sanitizer, acne pads or any product containing alcohol – these items are not allowed on-site and will not be stored.               
  • Prescription or over-the-counter medication that is not allowed on-site (as reviewed at the time of your application).
  • Clothing with explicit messages or images, does not cover undergarments or does not reach your mid-thigh or cover your midriff.
  • Protein powder or supplements for weight loss or muscle enhancement.
  • Please avoid bringing valuables and sentimental items.
  • We ask that you not carry more than $50 cash.
  • We are a scent-sensitive environment – perfume, cologne, body spray, perfume body lotion, and other scented products are not allowed.
  • Books, movies, or magazines that contradict recovery or are sexually explicit.
  • Knives or other items considered to be weapons (as determined by the program).
  • Other items may be secured if deemed inappropriate by the program.
  • Aerosols; unscented products in a pump dispenser is allowed if alcohol-free.

HOW DO I APPLY?

You will find our Application on our website Here.  You can complete and submit your application through our website, or print it off and return by mail or fax.  You can also drop a completed application off at Crossroads Centre. 

Do I need to be referred by a service provider?

No.  You can self-refer by completing an application.  If you are working with an agency, your worker can complete the referent information page in the application if you prefer. 

Do you provide detox services?

No, we do not provide on-site withdrawal management.  We will support you to access this service in Thunder Bay if needed prior to admission.

Are you a treatment program? What is the difference between Crossroads Centre and treatment?

Crossroads Centre is a residential recovery program that provides abstinence-based services before and after treatment.  Our program provides individual and group programming prior to attending or after completing a treatment program.  Length of residency before or after treatment is based on individual program planning. 

How long is the program at Crossroads?

The length of residency at Crossroads will be determined by your personal treatment plan.  Depending on when you are scheduled to attend treatment, pre-treatment can range from a few weeks to a few months.  Post-treatment residency is individualized based on your recovery goals and can range from a few months to a year or more.

When will I hear about my application?

Once your application is received, Crossroads will contact you to confirm it has been received and to let you know if any additional information is needed to process your application.  An intake interview will be scheduled prior to admission.  

If there is a waitlist at the time you apply you are responsible to contact Crossroads once a week to check-in.  This weekly check-in will hold your spot on the waitlist.

How long is the waitlist?

The length of wait time for services varies, and a bed can become available at any time.  To reduce any delay with your application and admission planning, please ensure your application is completed in full and that any additional information we require is provided promptly.  

Is there a cost to attend Crossroads?

No, Crossroads is fully funded by the Ministry of Health and there is no cost for our services, accommodation or meals.  Any personal items and purchases not related to what is covered by the program, is your responsibility (toiletries, clothing, etc.). 

What medications can I bring?

There are prescription and over the counter medications and products that are not allowed at Crossroads Centre.  We will review this information with you during the intake process, and again prior to admission.  

Crossroads Centre does allow methadone and suboxone.  These medications are administered off-site and you must go daily to the pharmacy or prescribing clinic for administration.

You are required to have 4 weeks of prescriptions for any medications upon admission.  Upon admission, Crossroads will arrange to have your prescriptions transferred to our pharmacy if this is different than your current pharmacy.   

With the exception of creams or ointments, do not bring open medication, prescription or over-the counter, as these cannot be administered.

What can I bring?

We have a detailed list of items and belongings that you will need to bring, or are welcome to bring, with you at admission.  Most rooms are shared with one other peer and the amount of personal belongings you can bring is limited to 2 bags.

What can’t I bring?

We have a detailed list of items that are not allowed on-site (belongings, toiletries, over-the counter medication, etc.).  Crossroads is a scent-sensitive environment and scented products are not allowed on-site (i.e. perfume, cologne, body spray).
